Do I need a physical address to rank locally?
For the map pack, Google requires a genuine address or a defined service area — and for service-area businesses the address can be hidden. What you cannot do is invent addresses in areas you want to rank in. That is against Google guidelines and profiles get suspended for it.
How many location pages should I have?
As many as you can make genuinely useful, and no more. Fifty near-identical pages with the place name swapped will not rank and can drag down the rest of the site. Ten pages with real local substance will outperform them comfortably.
